This process will take place around October or November. The membership will automatically be extended every year. This permission will continue until deregistration. The deadline for deregistration is August 31st and has to be done by filling in the deregistration form on the website and sending this to secretary@studyassociationpolis.com. If this is not done before August 31st, the membership will be extended by one year. The deregistration is only valid when I receive a confirmation from the secretary. By giving permission, I will make sure that I have enough money on my bank account so the direct debit will succeed. I am obligated to report any changes of my personal information (this can be done on the website). The direct debit will take place according to the SEPA-guidelines. You can resign within four weeks after filling in this form by sending an e-mail to secretary@studyassociationpolis.comBy signing this form I am aware of and I approve the privacy conditions of Study Association POLIS.
